Care Transition Associate

Position Summary The Care Transition Associate performs a variety of patient-centered administrative tasks to support care coordination within acute care hospital hubs. This role focuses on post-acute care needs for patients requiring home health and hospice, supporting patients and families with their discharge plan and ensuring a timely start of care. The Associate facilitates communication between hospital staff and post-acute service providers, and may collaborate with local Service Line leadership on training, orientation, and tactical reporting. Key Responsibilities Act as a non-clinical liaison between inpatient and outpatient settings, supporting post-acute service arrangements and coordinating care for patients discharging with Home Health and Hospice needs. Assist Clinical Liaisons and inpatient acute care teams in arranging care, obtaining clinical information, and ensuring accurate charting and documentation. Coordinate admissions to home health and hospice service, ensuring that services, equipment, and documentation meet clinical criteria and insurance requirements. Maintain excellent customer relationships with Clinical Liaisons, discharge planners, physician groups, and referral sources; provide updates and educational materials to patients and families. Coordinate equipment delivery to patient residences with HME and DME vendors to avoid discharge delays. Conduct administrative support for the Clinical Liaison team, enter patient data into EMR, and schedule starts of care. Manage referral and reimbursement processes, follow up on orders, and communicate changes in patient condition. Maintain HIPAA-compliant handling of confidential information and adhere to all regulatory requirements. Participate in scheduled meetings with the Clinical Excellence Team and stay current on managed care contracts and network provider subcontracts. Perform additional duties as assigned. Qualifications Associate’s Degree or equivalent education/experience (high school diploma plus three years of work experience is equivalent). Demonstrated work experience with patients and families and customer service in a health‑care setting. Proficiency with MS Word, Excel, and Epic electronic medical records (EMR) preferred. Strong written and verbal communication skills in English. Mathematical proficiency for calculations of rates, ratios, and percentages. Ability to read, analyze, and interpret business and technical documents. Knowledge of medical terminology preferred. Collaboration and relationship-building with diverse stakeholders. Critical thinking and problem‑solving skills. Computer literacy and proficiency with electronic medical records, fax, and copy machines. Understanding of the Value‑Based Enterprise agreement between Providence and JV for home health and hospice care coordination. Physical Demands & Work Environment The role requires regular standing, walking, and manual dexterity, including lifting and moving objects up to 25 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. The position involves interpersonal communication with patients, families, and healthcare teams.
