Licensed Practical Nurse (LPN) – Home Health

Schedule: Monday–Friday, 8:00 AM–5:00 PM, with evening and weekend on-call rotation

Coverage Area: Harrison, Marion, Tyler, Barbour, Doddridge, Lewis, and Upshur Counties

Mileage Reimbursement: $0.54 per mile

Productivity Requirement: 32 points per week

Position Overview

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Home Health team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring their medical needs are met while promoting independence, dignity, and quality of life. The ideal candidate will have prior home health experience and be proficient with HCHB and OASIS documentation.

Responsibilities

• Deliver nursing care in accordance with the patient's plan of care and physician orders.
• Monitor and document patient vital signs, symptoms, and overall condition, reporting abnormal findings as appropriate.
• Administer medications, treatments, feedings, oxygen therapy, ostomy care, and other nursing interventions as required.
• Collaborate with physicians, therapists, and other healthcare professionals to support patient care goals.
• Carry out treatment plans established by Physical Therapists, Occupational Therapists, and Speech Therapists.
• Maintain strict infection control standards and report any suspected communicable diseases or infections.
• Accurately document patient care, progress, outcomes, and communications in a timely manner.
• Order and monitor supplies, medications, and nutritional feedings to ensure patient needs are met.
• Monitor patient incidents, complete required reports, and communicate with appropriate personnel regarding concerns.
• Inspect and monitor medical equipment, reporting malfunctions and coordinating repairs as needed.
• Ensure all patient care is delivered in compliance with regulatory requirements and organizational standards.

Qualifications

• Current West Virginia Licensed Practical Nurse (LPN) license required.
• Previous home health experience required.
• Experience with Homecare Homebase (HCHB) and OASIS documentation required.
• Strong clinical assessment and patient care skills.
• Excellent communication, documentation, and organizational abilities.
• Ability to work independently while managing a designated territory and patient caseload.
• Valid driver's license, reliable transportation, and appropriate auto insurance required.
