Navigate Care is seeking a Virtual Nurse Practitioner to conduct brief telehealth visits for Medicare patients, focusing on care-navigation. You will determine medical necessity, document relevant data, and create tailored care plans for ongoing support. Requires an accredited NP program, Texas NP license, national board certification, PECOS enrollment and 2+ years in relevant fields.
