Travel Physical Therapist - $2,046 per week

Overview Centra Healthcare Allied is seeking a travel Physical Therapist for a travel job in Scottsdale, Arizona. Responsibilities Assess referred patients requiring rehabilitation services; provide direct patient care to evaluate medical condition, functional capabilities, limitations and restrictions, and potential for rehabilitation. Establish and administer a treatment program with specific goals determined by the patient’s capacity and tolerance under the direction of the Physician. Set realistic and achievable goals for patients, document and record the patient’s condition, and educate patients and families in an appropriate physical therapy method. Collaborate with all disciplines to plan and evaluate team goals for each patient. Requirements Must have graduated from an accredited school. Current state license as a Registered Physical Therapist ( PT ) or in the process of licensure. Current CPR certification. Develop a plan of care for each physical therapy patient. Provide skilled physical therapy services/interventions in accordance with physician orders. Assess patient needs, plan for, evaluate and modify care to meet goals of physical therapy interventions. Collaborates with all disciplines to plan and evaluate team goals for each patient.
