Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Start Date: 09/21/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Job Summary: Physical Therapist (PT) specializing in Home Health (HH), located in the specified area, responsible for planning and conducting segments of treatment programs designed to restore and improve patients' physical, social, and mental functions. Serves as a co-leader within an interdisciplinary team to deliver personalized in-home care that aligns with departmental objectives. Job Details: Schedule: Day shift, 8 hours per day, with a minimum of 40 hours per week. Required Qualifications: Doctor of Physical Therapy (DPT) degree required; only if obtained license on or after January 2016, or foreign equivalent. Valid license to practice physical therapy in all applicable states. Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS or HS-BLS or RQIBLS) certification from an approved vendor per policy. Valid Drivers License (DL) and must be at least 21 years of age or older. Additional Information: Completes evaluations to ensure quality standards and addresses assessment areas requiring attention. Uses appropriate clinical techniques and tools, establishing individualized treatment plans for patients. Provides therapy interventions using approved protocols and techniques. Establishes individualized care and discharge plans, communicating these plans to relevant parties. Completes comprehensive and accurate clinical documentation within established deadlines. Supervises assigned assistants. Performs other job-related duties as assigned.
