You will work closely with an interdisciplinary team to deliver personalized, patient-centered care.### **Key Responsibilities*** Perform **patient evaluations** and develop individualized treatment plans* Provide therapy to improve **mobility, strength, balance, and functional independence*** Monitor patient progress and adjust treatment plans as needed* Maintain accurate and timely **clinical documentation*** Communicate effectively with physicians, residents, families, and care team members* Participate in **care planning and discharge planning*** Conduct **staff education and in-service training*** Supervise **PTAs, aides, and students** as needed### **Qualifications*** Degree in **Physical Therapy** (Bachelor’s required; Master’s or Doctorate preferred)* Active, unencumbered **Physical Therapist license in South Carolina (required)*** Experience in a **skilled nursing or rehab setting preferred*** Familiarity with **PointClickCare (PCC) and Casamba preferred*** Strong clinical, communication, and leadership skills* Commitment to patient-centered care### ### ### **Why Join Cherrydale Post Acute?**At Cherrydale, you’ll enjoy the flexibility of PRN work while being part of a **team that values your expertise and supports your growth**.
