Licensed Practical Nurse (LPN)

This position involves providing nursing care in a Long-Term Care (LTC) or Skilled Nursing Facility (SNF). The role requires collaboration with a multidisciplinary team to ensure the highest standards of care for residents, including those with skilled, rehab, and dementia diagnoses.

Shift Length: 8 hours

Shift Time: 2:45 PM to 11:00 PM, every other weekend required

The Licensed Practical Nurse (LPN) will be responsible for the following:

• Administering medications and treatments as prescribed
• Monitoring resident health, including vital signs and reporting changes
• Providing assistance with activities of daily living (ADLs)
• Conducting lab draws and specimen collections
• Managing wound care, including measuring wounds
• Performing glucose monitoring and insulin administration
• Documenting care and assessment findings in the Electronic Medical Records (EMR) system (PCC)

Candidates must meet the following requirements:

• Current LPN License
• BLS Certification
• Minimum of 1 year of experience in a Long-Term Care setting within the last 3 years
• Experience with wound measurement and familiarity with PCC EMR

The work environment includes a long-term care facility with approximately 300 total beds across six units. Staffing includes a Nurse Manager, supervisors, charge nurses, and floor nurses (both LPNs and RNs). The facility operates without tele monitors and has nursing support for various medical and rehabilitation needs.

• Orientation provided on Tuesdays only. Clearance required before the start date.
• Nurses must use the facility's time clock and follow badge protocols.
• Flexible scheduling may be available, with consistency in assigned areas encouraged.
