Position Title: Licensed Practical Nurse (LPN) - Home Health
Location: Coverage Area Includes Grady, Decatur, Mitchell, Seminole, and Early Counties
Schedule: Monday - Friday, 8:00 AM - 4:30 PM
Hours: 40 hours per week
On-Call/Weekend Requirement: None
Mileage Reimbursement: Yes, between patient visits
Dress Code: No specific scrub color required

Position Overview

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our Home Health team. In this role, you will provide skilled nursing care to patients in their homes while working closely with an interdisciplinary healthcare team. The ideal candidate will have prior home health experience, strong clinical assessment skills, and a commitment to delivering high-quality patient-centered care.

Key Responsibilities

• Provide high-quality clinical services within the LPN scope of practice in accordance with state nursing regulations and established plans of care.
• Deliver skilled nursing services in patients' homes while adhering to all infection prevention and control protocols.
• Monitor patient conditions and promptly report changes in health status to the appropriate healthcare team members.
• Thoroughly document all patient visits in the electronic medical record in compliance with regulatory, agency, and payer requirements.
• Utilize the agency's electronic visit verification (EVV) system and documentation platforms as required.
• Participate actively in care coordination activities, including case conferences and interdisciplinary collaboration.
• Communicate effectively with patients, families, physicians, case managers, and other members of the healthcare team.
• Educate patients and caregivers regarding treatment plans, disease management, medications, and available resources.
• Provide compassionate care to patients of all ages and backgrounds while demonstrating respect for diverse cultures, beliefs, and circumstances.
• Ensure compliance with all company policies, state regulations, and home health standards of practice.

Required Qualifications

• Current and unrestricted Licensed Practical Nurse (LPN) license.
• Previous home health experience.
• Experience using Homecare Homebase (HCHB) documentation software.
• Strong clinical, organizational, and time-management skills.
• Valid driver's license, reliable transportation, and the ability to travel throughout the assigned coverage area.
