Nurse Practitioner

Role Overview Signify Health, part of CVS Health, is hiring part‑time board‑certified nurse practitioners to conduct In‑home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. Responsibilities Conduct pre‑scheduled in‑home visits with health plan members, traveling primarily locally within a designated metro area (e.g., Springfield, MA). Provide a minimum of 12 hours of availability weekly, consistently completing 50+ capacity units per month. Maintain a strong visit completion rate, pacing 35+ completed visits per month. Provide availability Monday‑Sunday, with the majority of visits scheduled between 8 a.m. and 7 p.m. Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s). Be accountable to Signify Health operational, clinical, and member experience key performance indicators and metrics. Conduct video visits as needed and maintain training, affiliation, and compliance with video visit requirements. Maintain training and complete all diagnostic and preventive service (DPS) products as directed. Maintain all required supplies and associated diagnostic equipment related to DPS offerings. Maintain active state licenses in coverage areas; FNP or AGNP certification preferred for NPs. Maintain board certification and multi‑state licenses (preferred). Possess experience with Signify Health In‑home Health Evaluations, in‑home visits, or a similar format (preferred). Be flexible to travel locally and within licensed states, as needed (up to 25%). Ability to sit, stand, bend, reach, push, pull and lift up to 20 pounds of supplies. Accommodate field observations and/or “ride‑along” experiences as requested. During evaluations, conduct a medication and medical history review, a physical evaluation, and any additional diagnostic tests ordered by the health plan. Qualifications Board‑certified nurse practitioner, with active state license(s). FNP or AGNP certification preferred. Experience with in‑home health evaluations or similar formats. Ability to travel locally within licensed states; up to 25% travel for remote locations. Experience with DPS training and supplies maintenance. Benefits Pay Range: $46.03 – $99.14 per hour, plus incentive structure (per visit completed and other incentives).
