Contract | Days | Monday–Friday, 8:00 AM–4:00 PM | Onsite

Key Responsibilities

• Provide occupational therapy services across a split setting: approximately 50% Home Health and 50% Skilled Nursing Facility, with flexibility based on area need
• Utilize setting-specific software platforms for documentation and care management
• Meet productivity expectations established for each care setting
• Collaborate with an onsite Therapy Supervisor and interdisciplinary care team to support patient outcomes

Required Qualifications

• Active Occupational Therapist (OT) license in the state of Minnesota
• Ability to work across both Home Health and Skilled Nursing Facility settings as needed
• Proficiency or ability to learn EMR platforms (HCHB for Home Health; NetHealth for SNF)
• Must reside within 80 miles of the facility to be considered a local placement

Preferred Experience / Skills

• Prior experience in Home Health and/or Skilled Nursing Facility settings
• Strong adaptability and flexibility to shift between care environments based on patient and facility need
• Familiarity with productivity standards in therapy-based settings
