Case Coordinator

As a Case Coordinator, you will play a vital role in supporting the success of our residential programs by coordinating services, managing documentation, facilitating interdisciplinary communication, and ensuring compliance with individualized care plans. This position combines case management, program coordination, and direct participant support to help individuals achieve their personal goals while ensuring exceptional quality of care. Case Management & Service Coordination Coordinate admissions and assist with the onboarding of new residents. Conduct 3-Day Preliminary Needs Assessments for new admissions. Develop Social Summaries and Individual Preferences, Desires, Needs, and Strengths documents, updating them annually. Assist with resident transitions to adult placements and other community resources. Conduct routine visits to residential homes to monitor services and provide ongoing support. Participate in community activities and events with residential programs. Program Planning & Documentation Develop, implement, and maintain individualized programs for residents. Create and update program data collection tools and tracking documents. Complete and distribute progress notes and ensure all documentation is maintained in program books. Perform annual updates to individualized programs and Policy & Procedure manuals. Conduct monthly audits of program documentation and communicate findings to leadership. Maintain accurate residential data and tracking reports using Microsoft Excel and other documentation systems. Keep caseworker assignments and contact information current. Meetings & Team Collaboration Schedule and coordinate admission, annual, 30-day, and interdisciplinary team meetings. Facilitate admission meetings and lead the residential portion of interdisciplinary meetings. Record and distribute meeting minutes to appropriate team members. Prepare and distribute House Manager updates before annual meetings. Submit required monthly Standing Committee reports. Represent the program during court hearings and external meetings with families, attorneys, social services, care coordinators, and other community partners. Training & Quality Assurance Provide training on annual programs and other program initiatives as assigned. Support quality improvement efforts through documentation reviews and compliance monitoring. Collaborate with residential staff and interdisciplinary teams to promote person-centered services and regulatory compliance. Required Qualifications High school diploma or GED required. Associate's or Bachelor's degree in Human Services, Social Work, Psychology, Rehabilitation, or a related field preferred. Minimum of two years of experience providing direct or indirect support to individuals with disabilities in a healthcare, rehabilitation, residential, or human services setting. Strong organizational, communication, and documentation skills. Ability to manage multiple priorities while maintaining attention to detail. Proficiency with Microsoft Office, particularly Excel. Ability to travel locally as needed. Preferred Skills Experience coordinating interdisciplinary services. Knowledge of person-centered planning and individualized service plans. Experience working with individuals with brain injuries, developmental disabilities, or neurological conditions. Ability to work independently while collaborating effectively with multidisciplinary teams.
