Clinical Pharmacist

Adult Inpatient Clinical PharmacistRequired Skills & Experience Doctor of Pharmacy (PharmD) degree. Active Pharmacist license in South Carolina. Previous inpatient experience is preferred but clinical inpatient rotations experience or 1 year of residency will be accepted. If no inpatient experience, retail management experience will be considered. EPIC experience is preferred.Nice to Have Skills & Experience - Clinical inpatient pharmacy experience.Job Description The Adult Inpatient Clinical Pharmacist is responsible for providing comprehensive pharmaceutical care to hospitalized adult patients. This role collaborates with physicians, nurses, and other healthcare professionals to optimize medication therapy, improve patient outcomes, promote medication safety, and ensure compliance with regulatory and organizational standards. The pharmacist serves as a clinical resource for medication management, participates in interdisciplinary rounds, and supports quality improvement initiatives throughout the hospital.Key Responsibilities Clinical Pharmacy Services Review, verify, and monitor medication orders for accuracy, safety, and therapeutic appropriateness. Assess and optimize pharmacotherapy for adult acute care patients. Provide dosing recommendations for antibiotics, anticoagulants, renal-adjusted medications, and other high-risk therapies. Perform pharmacokinetic monitoring and therapeutic drug management. Evaluate patients for medication-related problems and recommend evidence-based interventions. Participate in interdisciplinary patient care rounds and collaborate with providers regarding treatment plans. Medication Safety & Operations Verify medication orders and ensure compliance with hospital policies and regulatory requirements. Support medication-use safety initiatives and adverse drug event prevention. Assist with medication reconciliation upon admission, transfer, and discharge. Provide oversight of formulary compliance and therapeutic interchange programs. Respond to medication-related inquiries from physicians, nurses, and other healthcare team members. Patient Care Educate patients and caregivers regarding medications, expected outcomes, side effects, and adherence strategies. Support transitions of care efforts to ensure continuity of medication management across care settings. Assist with discharge medication planning and counseling as needed. Clinical Support & Education Serve as a drug information resource for healthcare providers. Educate pharmacy staff, residents, students, and other clinicians on medication management and best practices. Participate in department meetings, committees, and clinical initiatives. Quality Improvement Participate in medication safety, antimicrobial stewardship, and quality improvement programs. Contribute to development and implementation of clinical protocols, order sets, and evidence-based pharmacy practices. Support regulatory, accreditation, and compliance activities.
