To learn more, visit Role Scope : We are looking for a dynamic, service-oriented dietitian who is passionate about supporting elderly persons in their journey. The Dietitian will work within a multidisciplinary care team led by a primary care provider to ensure whole-person, integrated care. The Dietitian completes participant nutritional assessments and reassessments, screens and assesses participant’s nutritional status and uses pertinent data to plan and implement appropriate nutrition interventions, obtains participant food preferences and makes dietary modifications as needed, in addition to providing nutrition education to participants, caregivers and Habitat PACE staff. Core Responsibilities & Expectations for the Role Participant Care Develop and implement nutritional plan of care for participants that takes into account social circumstances and medical needs, in collaboration with the PCP and interdisciplinary team. Plan and execute follow ups and reassessment for each participant, or as necessary if there are changes in participant status. Assess participants’ access to food and makes referrals to interdisciplinary team for meal preparation evaluation and grocery shopping needs. Determine participants’ need for and order oral nutritional supplements accordingly. Provide instruction to participants and caregivers regarding dietary regimens and nutritional requirements (home visits as required). Develop and implement the nutrition care plans for high-risk participants including medical nutrition therapy recommendations. Monitor food intake, therapeutic diets and special dietary modifications and communicate information and concerns with the care team. Administration & Program Development Continue to raise the bar. Constructively seek and share feedback and help us implement changes in order to improve clinical outcomes and experience for participants. Conduct sanitation and safety checks for kitchen and kitchen equipment as needed. Monitor adequacy of breakfast, lunch and snack menus and notes/make adjustments to any dietary regimens or nutritional requirements based on individual participant’s needs. Update the meal calendar, supplement sheet, diet lists and assisted dining room guide as needed. Attend care planning sessions and report on nutritional status of participants. Work with the State and Federal Food Program as guided by regulations. Serve as onsite subject matter expert to support quality programs and discipline-specific audits. Required Qualifications: Master’s degree or equivalent in food science, nutrition, and/or dietetics as required by the credentialing body Current medical terminology and the ability to complete written assessments and document participant care. Nationally Certified as a Registered Dietitian Nutritionist. A minimum of one year experience working with the frail or elderly. Nice to have: 2+ years as a practicing Registered Dietitian. PACE experience and/or ADHC/CBAS experience. A state issued driver’s license, personal transportation, and auto insurance as required by law. Perform tasks that may involve bending, reaching, lifting, or assisting participants, depending on role responsibilities. Lift, carry, push, or pull supplies, equipment, or materials typically up to 25–50 pounds, with assistance or mechanical aids, depending on job duties and role responsibilities. Compensation: The anticipated base salary range for this position is $85,000 -$96,000 . This position is also eligible for a target annual bonus of 5% and may be eligible for additional incentive compensation, as applicable. The final compensation offered will be determined based on factors including, but not limited to, the candidate's qualifications, skills, experience, education, certifications, geographical location, and business needs. In addition, this position is eligible for a comprehensive benefits package, including medical, dental, and vision insurance; paid parental leave, short- and long-term disability coverage; life insurance; flexible spending accounts; a 401(k) savings plan that includes a match; paid time off; and company paid holidays.
