Job Description

Choice Health at Home is looking for a compassionate and skilled Licensed Vocational Nurse (LVN/LPN) to join our growing Home Health team. If you're passionate about providing one-on-one patient care, enjoy building meaningful relationships, and want the flexibility of working in the field, we'd love to meet you.

As a Home Health LVN, you'll help patients recover, manage chronic conditions, and maintain their independence—all from the comfort of their own homes. You'll work alongside experienced clinicians in a supportive, patient-first environment where your care truly makes an impact.

What You'll Do

Deliver skilled nursing care in patients' homes under the supervision of a Registered Nurse
Assist with patient assessments and contribute to individualized care plans
Administer medications and provide treatments as ordered
Perform wound care and monitor patient progress
Educate patients and caregivers on disease management, medications, and home care
Maintain accurate clinical documentation and communicate effectively with the interdisciplinary care team
Follow infection prevention and safety protocols
Ensure high-quality, compassionate care while promoting patient independence

What We're Looking For

Current LVN/LPN license
Valid driver's license and reliable transportation with current auto insurance
Current CPR certification
Home health experience preferred
HomeCare HomeBase experience is a plus
Excellent communication and organizational skills
A compassionate, patient-centered approach to care
