Job Title: Registered Nurse (RN) Case Manager Job Location: Kansas City, MO, Independence, North Kansas City Company: Specialized Home Care Job Summary We are seeking compassionate Registered Nurses (RNs) to join our team as Case Managers, working one-on-one with patients in their home environment. As a Case Manager, you will establish and oversee all aspects of patient care, creating a Plan of Care and following physician orders to achieve outcomes of increased patient independence and satisfaction.
