Licensed Practical Nurse Outpatient PACE Program

Position Overview Licensed Practical Nurse (LPN) – Outpatient Clinic PACE Program, CarePartners Health Services. Monday–Friday, closed weekends. Job Summary The LPN supports participants through comprehensive medication oversight, assessment and hands‑on care, serving as an essential member of an interdisciplinary team that promotes independence and quality of life in the community. Key Responsibilities Provides direct patient care in an outpatient setting. Evaluates and records patient vitals. Assists providers with therapeutic and diagnostic procedures. Administers oral and injectable medications as ordered and monitors patient responses. Delegates tasks to licensed and unlicensed staff within scope of practice and competency. Delivers quality customer service to patients and their families. Triages participants, addresses their needs and concerns, and makes clinical judgement to determine the necessity of physician evaluation. Coordinates care with specialist providers and nursing facilities. Collaborates with the care team to implement goals and interventions. Assesses participants’ medical needs, creates and supports care plans for community success. Co‑ordinates orders and interventions with SNF partners. Consults with patients, colleagues, and interdisciplinary teams to achieve desired outcomes. Performance Criteria Regularly documents and reports patient data, responses, and progress. Individualizes patient teaching and care plans. Advocates for patients and families to help achieve desired outcomes. Identifies problems and implements measures to address needs throughout continuum of care. Qualifications Basic Cardiac Life Support (BCLS) certification within 30 days of employment start date. LPN or LVN license within 2 years of employment start date. Associate degree in nursing. No travel required.
