What You'll Do

• Administer medications and treatments according to provider orders, applicable nursing standards, and community procedures
• Verify medication orders and maintain accurate medication administration records
• Observe residents before, during, and after medication administration
• Monitor residents for changes in physical condition, cognition, behavior, mood, appetite, mobility, and daily functioning
• Obtain and document vital signs and other assigned resident health information
• Provide direct nursing care and resident support within the Virginia LPN scope of practice
• Assist residents with activities of daily living when nursing or resident-care support is needed
• Collect, document, and communicate resident information to the appropriate nursing leader
• Promptly report significant changes in resident condition
• Initiate appropriate interventions within the LPN’s education, competency, scope of practice, and assigned responsibilities
• Notify the appropriate nursing leader, provider, family member, or emergency service when required
• Participate in resident assessments and service-plan reviews under appropriate clinical direction
• Assist with implementing, evaluating, and maintaining individualized resident service plans
• Receive, review, and carry out provider orders within the LPN’s scope of practice and community procedures
• Coordinate medication refills and communicate with pharmacies and providers
• Document medications, treatments, observations, incidents, and nursing services accurately and promptly
• Perform medication-cart, treatment-record, resident-record, and documentation audits as assigned
• Support resident admissions, transitions, hospital returns, and changes in condition
• Provide resident and family teaching within the LPN’s training, competency, and scope of practice
• Promote wellness, hydration, nutrition, infection prevention, medication safety, and fall prevention
• Respond promptly to resident emergencies and urgent needs
• Participate in resident-care meetings and communicate relevant information to the Wellness team
• Provide day-to-day direction, supervision, and support to resident-care staff as assigned
• Reinforce resident service plans, care instructions, and community procedures
• Assign or delegate resident-care activities only when permitted by law, community policy, staff competency, and the resident’s condition
• Monitor the completion and effectiveness of assigned or delegated activities
• Assist with staff education, competency reviews, audits, and required in-service training
• Maintain resident privacy, dignity, rights, and confidentiality
• Complete additional nursing and Wellness duties within the LPN’s lawful scope of practice

What We're Looking For

• Current and valid Virginia LPN license or valid multistate nursing privilege authorizing practice in Virginia
• License or practice privilege must be active and in good standing
• Knowledge of practical nursing standards, medication administration, infection prevention, and resident safety
• Strong observation and resident-monitoring skills
• Ability to recognize, document, and report changes in resident condition
• Ability to work within the LPN scope of practice and seek RN or provider guidance when required
• Strong documentation, organization, and time-management skills
• Comfortable using electronic health records or resident-care documentation systems
• Ability to communicate professionally and compassionately with residents, families, providers, pharmacies, and coworkers
• Ability to communicate clearly and effectively in English, verbally and in writing, as required to understand provider orders, document nursing services, administer medications safely, report changes in condition, and respond during emergencies
• Ability to provide appropriate direction, supervision, and support to resident-care staff
• Ability to work independently while recognizing when a concern must be escalated
• Current CPR or Basic Life Support certification, if required by the community
• Previous assisted living, memory care, long-term care, home care, or senior-care nursing experience is preferred
• Previous experience working with older adults is preferred
• Dependable attendance and punctuality
• Ability to stand, walk, bend, reach, and remain active throughout the workday
• Ability to safely lift, transfer, push, or pull up to (physical requirement) pounds with assistance or available equipment
• Ability to successfully complete required background checks, health screenings, and employment requirements

Virginia Nursing Requirements

• Must maintain an active Virginia LPN license or valid multistate privilege throughout employment
• All nursing services must be performed within the Virginia LPN scope of practice
• Nursing duties must be consistent with the resident’s condition, provider orders, the LPN’s education and competency, clinical direction, and community procedures
• Responsibilities requiring independent RN assessment, advanced nursing judgment, or another licensed provider must be referred or escalated appropriately
• Assignment and delegation decisions must consider the resident’s condition, the nature of the task, the employee’s qualifications and demonstrated competency, and the supervision available
• The nurse must provide appropriate direction, monitoring, evaluation, and follow-up for assigned or delegated activities
• Medication administration and nursing documentation must comply with provider orders, applicable Virginia requirements, and the community’s medication-management plan
• The employee may only perform specialized procedures for which they are legally authorized, properly trained, determined competent, and assigned
• Significant changes, unstable conditions, medication concerns, or duties outside the LPN’s scope must be promptly escalated to the appropriate nursing leader, provider, or emergency service
