Key Responsibilities: Provide psychosocial assessments, counseling, and support to patients and their families navigating medical treatment Collaborate with physicians, nurse practitioners, and care coordinators to integrate emotional and social care into treatment planning Help patients overcome barriers to care, including transportation, housing, insurance, and caregiver needs Offer crisis intervention, grief counseling, and guidance through difficult care transitions Qualifications: Licensed Clinical Social Worker (LCSW) in the state of New York 3+ years of experience in medical, oncology, or grief/bereavement counseling preferred Strong interpersonal skills with the ability to build rapport across interdisciplinary teams Passion for holistic, patient-centered care and a desire to lead new initiatives Why This Role?
