Advanced Practice Provider (APP) - In-Home Primary Care

Job Description VillageMD is currently seeking a Nurse Practitioner or Physician Assistant to join our home-based primary care team to support our vulnerable Medicare and Medicare Advantage patients in Griffin, GA. Travel will be required to other nearby cities, including but not limited to Lithonia, McDonough, and Monticello. We Offer Access to leaders and knowledge-sharing opportunities across a national network of colleagues who are leading the transformation of primary care Reduced administrative burden and support Easy-to-use, optimized EMR with full telehealth capability High earnings potential aligned with our innovative clinical model Sign‑On Bonus Eligible Personal and Professional Skills for Success Graduate and certified from an accredited advanced practice program Two or more years of home care clinical experience as an Advanced Practice Provider or Registered Nurse, with a demonstrated ability to work autonomously and provide care without physician supervision Comfortable treating adult patients and conducting comprehensive or episodic health history and physical assessments of patients/families with acute, chronic, and/or potential health problems Working knowledge of chronic care management and various medication options Current, unrestricted State license or eligibility for licensure and a valid driver’s license Active Drug Enforcement Agency (DEA) registration or eligibility for registration Must possess active Medicare/Medicaid enrollment or be eligible to enroll Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) Comfortable with Electronic Medical Record (EMR) systems; Athena preferred Familiarity with Microsoft Office Benefits Participation in VillageMD's benefit platform includes Medical, Dental, Life, Disability, Vision, FSA coverages, and a 401(k) savings plan.
