Description Job Title: Social Worker - Pediatrics (Ambulatory Care) Social Work Services at the Mount Sinai Hospital The Mount Sinai Hospital is a 1,134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. We believe that total patient care must emphasize the physical, emotional, and social needs of each patient and their care partners. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907, the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education, professional development and extra-curricular opportunities. The social worker in this grant funded position will provide complex care management and comprehensive social work services to children and their families under a grant whose mission is to provide access to social work services for the underserved. Social work services will include primary mental health evaluations and short-term treatment; complex care management and collaboration with community providers; supportive counseling; and psychoeducation to encourage adherence to primary care and adaption of family centered wellness behaviors. This social worker will also follow families who are at high risk for problematic parenting and will at times encounter families at risk of abuse/child maltreatment and collaborate closely with the child protection social worker. This social worker also collaborates with a large number of attending physicians, resident physicians, nurses, and administrators and will be an integral part of the pediatric practice. Responsibilities An ability to work flexibly and to set priorities is essential in this fast-paced setting. The ability to engage families during pediatric appointments and to work outside the confines of an appointment schedule is essential. This program will have a set of metrics about deliverable services, with maintenance of data on cases served as a key component. Qualifications
NYS LMSW/LCSW.
Demonstrated clinical skills, preferably with children and adolescents within ambulatory care populations. Strong teamwork and organizational skills. Ability to collaborate with a large group of medical providers. Ability to work with a large population with variable social work needs for interventions. Bilingual (English/Spanish) preferred. Minimum of one year in current position unless a Float in Women’s and Children’s. Compensation The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $88691 - $101670.78 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need.
