If you're passionate about patient-centered care and love the flexibility of home health nursing, we want to hear from you!Provide direct patient care in home settings, including wound care, medication administration, and vital sign monitoring. Develop and follow individualized patient care plans in coordination with the supervising RN and care team. Observe, document, and report changes in patient condition to appropriate clinical staff. Administer prescribed medications, injections, and treatments per physician orders. Educate patients and family members on care procedures, medications, and health management. Maintain accurate and timely clinical documentation in compliance with state and agency standards. Coordinate with multidisciplinary care teams to ensure continuity and quality of patient care.Active Licensed Vocational Nurse (LVN) license in the state of California. Current CPR/BLS certification. Prior home health, skilled nursing, or clinical care experience preferred. Strong knowledge of wound care, medication management, and patient assessment. Excellent communication, time management, and organizational skills. Reliable transportation and valid California driver's license with proof of insurance.
