To support quality outreach initiatives, the part-time Quality Outreach Coordinator will conduct clinical outreach calls, manage member records, and assist in closing care gaps for high-risk members in a completely remote environment. Key responsibilities Conduct incoming and outgoing calls to educate members on closing gaps in care and provide health education materials Maintain accurate records of outreach efforts, member issues, and barriers to care Act as a liaison between members, providers, and community resources to facilitate appointment scheduling and support Required qualifications Associate's degree in a healthcare field or 3 years of relevant work experience 2 years of experience in case management, community health, or a health insurance environment Minimum of 2 years of experience working with members to close gaps in care 1 year of managed care experience Knowledge of the Managed Care or Health Insurance Industry and medical terminology
