About NYS Veterans Homes: New York State Veterans Homes are skilled nursing facilities owned and operated by the New York State Department of Health for veterans and their dependents. We provide quality care to all eligible veterans and their eligible dependents in need of skilled nursing care and rehabilitative services. We also conduct geriatric research and provide education and training for health care professionals. The Department of Health operates four Veterans Homes, located in Batavia, Oxford, Montrose and St. Albans (Queens), New York. (State University of New York (SUNY) operates a fifth Veterans Home in Long Island, New York) Responsibilities: Regulatory Compliance and Quality Assurance - Ensures adherence to all applicable State and federal laws, rules, and regulations, as well as facility policies related to restraints, seclusion, intensive supervision, abuse prevention, and incident reporting. - Develops, implements, and monitors corrective action plans in response to identified regulatory deficiencies to ensure timely and effective resolution. - Must have thorough knowledge of DOH, CMS, and federal regulations. - Demonstrated leadership experience with the ability to manage staff confidently and professionally. - Understands the legal ramifications of failing to provide necessary care to veterans - Must be knowledgeable about survey processes and requirements. - Must be competent in admissions, clinical assessment and documentation, care planning, completion of the GG (Minimum Data Set) assessments, and reporting cases to the Department of Health (DOH) in accordance with regulations. Regulatory Compliance: - Ensure compliance with: NYS Department of Health (DOH) and Federal CMS regulations. Always maintain survey readiness, Prevent deficiencies related to: Documentation, Infection control and Medication management Supervisory Responsibilities: - Supervises and coordinates the activities of assigned nursing and direct care staff. - Direct and reviews the work of nursing staff; completes performance evaluations of assign staff; and initiates appropriate counseling or disciplinary actions when necessary. - Develops assignments and daily staffing plans for nursing care; prepares and monitors staffing schedules; and ensures adequate nursing coverage. - Identifies staff development needs; develops, presents, and/or arranges for training on clinical nursing topics; and evaluates the effectiveness of in-service education and guidance programs. - Ensures that staff meet requirements for first aid, cardiopulmonary resuscitation (CPR), and other agency-specific training required. - Participates in the recruitment process, including interviewing nursing candidates, participating in orientation programs for new staff, and other hiring activities. Clinical and Operational Oversight: - Supervises admission and discharge planning activities; serves as a liaison to community providers, families, hospitals, and other healthcare facilities. - Maintains established standards of nursing care in accordance with State, federal, and accreditation requirements. - Makes rounds to ensure a safe, sanitary, and therapeutic environment; ensure that residents’ safety, well-being, and care needs are met. - Communicates and enforces policies, procedures, and current trends in nursing practice. - Monitors and reviews nursing documentation, such as progress notes and medication charts, for accuracy and quality; follows up with staff to resolve discrepancies. - Assesses the quality of nursing care and assists with developing and implementing quality control programs. - Oversee narcotics tracking and management; ensures that medical inventories are properly maintained and stored. Administrative Participation: - Participates in the management and administration of nursing services, including planning and program development. - Meets with managers to review, update, and ensure the effectiveness of nursing policies, protocols, and procedures. - Assists with department oversight, including budget preparation, expenditure monitoring, and committee/workgroup participation. - Performs the duties of a Registered Nurse 1 and covers staff assignments when necessary. Medication & Treatment Management: - Oversee medication administration practices Ensure, No expired or discontinued medications, Proper documentation, then Investigate and report medication errors Staff Supervision & Leadership: - Supervise RNs, LPNs, and CNAs on assigned shifts, Assign duties and ensure proper coverage, Provide guidance, coaching, and support, Address: Performance issues, Unprofessional behavior and Policy violations Staffing & Scheduling Oversight: Ensure adequate staffing for safe care delivery, Manage: Callouts, Shift coverage, Overtime needs and Reassign staff as needed based on acuity Documentation & Care Planning: Ensure accurate and timely documentation, Review: Nursing notes, Incident reports, Care plans both baseline and comprehensive, Ensure proper use of GG documentation and ADL charting Admissions & Discharges: - Oversee and/or complete: Admission assessments, Physician notifications, Orders and care plan initiation and Ensure timely documentation and coordination of care Infection Control: - Enforce infection prevention protocols, Monitor outbreaks isolation precautions and EBP Educate staff on: PPE use, Hand hygiene, Cohorting when necessary Incident & Risk Management: - Ensure all incidents are: Reported immediately, Investigated and documented. Follow up on: Falls, Abuse allegations, misappropriation, neglect and so on Time & Attendance Oversight: -Verify staff attendance and productivity, Ensure staff are: Working while on the clock and Not engaging in unauthorized overtime Address: Tardiness and Absenteeism promptly Education & Quality Improvement: - Ensure staff complete: In-services and Competencies - Participate in: (QAPI) Quality assurance and performance improvement Note: The RN Supervisor is accountable for ensuring safe resident care, staff performance, regulatory compliance, and operational efficiency during their shift.
