Home Health Licensed Practical Nurse (LPN)

Full-Time MondayFriday 8 a.m.5 p.m.
$38$42/hour based on experience No Weekends or On-Call

At Elara Caring were seeking a Licensed Practical Nurse to deliver compassionate skilled care in patients homes. Youll build meaningful relationships through one-on-one visits while working closely with an RN Case Manager and a supportive clinical team.

What Youll Do

• Provide skilled nursing care according to each patients individualized care plan and physician orders.
• Monitor patient progress and promptly report changes in condition or safety concerns to the RN Case Manager.
• Teach patients and caregivers techniques that support health safety and independence at home.
• Collaborate with the RN Case Manager as patient needs and care plans change.
• Document visits nursing interventions and medication information accurately and promptly.
• Communicate with physicians patients families and the clinical team to keep care coordinated.
• Help reduce avoidable hospitalizations through attentive care and early recognition of concerns.

What Youll Bring

• An active Massachusetts LPN license
• Graduation from an accredited nursing program
• At least one year of clinical nursing experience
• A valid drivers license reliable transportation and current auto insurance
• Ability to travel between patient homes and assigned work locations
• Ability to sit stand bend lift and move intermittently including lifting 50100 lbs.
