RN: Case Manager

RN: Case Manager Hattiesburg, MS Responsibilities Perform comprehensive assessment of high risk members. Collaborate with primary care providers to implement individualized care plans. Address, identify, and reassess appropriate levels of care. Coordinate care transitions with physicians, social workers, and discharge planners. Maintain detailed records of clinical, functional, and fiscal outcomes. Identify opportunities for health promotion and illness prevention. Act as patient advocate by protecting privacy and confidentiality issues. Work evenings, M-F 8:00 a.m.–5:00 p.m., and be on call every other week Mon‑Sun. Perform actions onsite at the work location. Education / Licenses Needed Associates or Bachelor’s Degree in Nursing or related field. Current, unrestricted state Registered Nurse license.
