MLR is seeking a highly skilled and dedicated Clinical Pharmacist to join our team at a well-regarded medical center. In this vital role, you will work closely with our healthcare team to ensure optimal pharmaceutical care, providing safe and effective medication management for our patients.

Key Responsibilities:

• Collaborate with physicians, nurses, and other healthcare providers to develop personalized medication therapy plans tailored to each patient's needs.
• Perform thorough medication reviews, assess potential drug interactions, and recommend interventions to enhance therapeutic outcomes.
• Offer drug information, advice, and education to healthcare staff, patients, and families to ensure understanding of treatment plans.
• Monitor patient responses to medications, make necessary adjustments to doses, and advise on potential side effects or adverse reactions.
• Participate in medication reconciliation processes and contribute to medication safety initiatives aimed at improving patient care.
• Help develop and implement evidence-based pharmacy protocols, guidelines, and best practices.
• Ensure compliance with all regulatory standards, policies, and procedures regarding medication management.
• Mentor and train pharmacy students, residents, and staff to foster professional growth and excellence in practice.

Qualifications:

• Preferred : Doctor of Pharmacy (Pharm.D.) degree from an accredited college of pharmacy.
• Active pharmacist license in NM.
• Preferred : Minimum of 2 years of clinical pharmacy experience in a hospital or healthcare setting.
