In this vital role, you will work closely with our healthcare team to ensure optimal pharmaceutical care, providing safe and effective medication management for our patients. Key Responsibilities:
- Collaborate with physicians, nurses, and other healthcare providers to develop personalized medication therapy plans tailored to each patient's needs.
- Perform thorough medication reviews, assess potential drug interactions, and recommend interventions to enhance therapeutic outcomes.
- Offer drug information, advice, and education to healthcare staff, patients, and families to ensure understanding of treatment plans.
- Monitor patient responses to medications, make necessary adjustments to doses, and advise on potential side effects or adverse reactions.
- Participate in medication reconciliation processes and contribute to medication safety initiatives aimed at improving patient care.
- Help develop and implement evidence-based pharmacy protocols, guidelines, and best practices.
- Ensure compliance with all regulatory standards, policies, and procedures regarding medication management.
- Mentor and train pharmacy students, residents, and staff to foster professional growth and excellence in practice.
- Preferred : Doctor of Pharmacy (Pharm.D.) degree from an accredited college of pharmacy.
- Active pharmacist license in NM.
- Preferred : Minimum of 2 years of clinical pharmacy experience in a hospital or healthcare setting.
