Physical Therapist

Adventist HealthCare Home Health seeks to hire a Physical Therapist servicing our Silver Spring Territory who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

As a Physical Therapist, you will:

• Performs an initial examination and evaluation to establish an impairment/diagnosis, prognosis and plan of care prior to intervention for all persons served with basic rehabilitation needs in acute care, outpatient or inpatient rehabilitation settings.
• Provides, or directs and supervises, the therapy intervention consistent with the results of the examination, evaluation, diagnosis, prognosis, and plan of care.
• The physical therapist, in consultation with appropriate disciplines, plans for discharge of the patient taking into consideration achievement of anticipated goals and expected outcomes, and provides for appropriate follow-up or referral.
• Communicates, coordinates and documents all aspects of patient management including the results of the initial examination and evaluation, diagnosis, prognosis, plan of care, interventions, response to interventions, barriers to progress, changes in patient status relative to the interventions, reexamination, and discharge/discontinuation of intervention and other patient management activities with members of the care team as indicated.
• Manages schedule to accommodate range in census and patient load and informs management of any issues with caseload management.
• Attends team meetings and effectively communicates patient status, using, if needed, the medical record or other treating therapists to ensure sufficient knowledge of the case.
• Responsible for individual professional development activities.

Qualifications include:

• Bachelors, Masters, or Doctorate in Physical Therapy from an accredited physical therapy program.
• 1-year site specific preferred.
• Active Therapy Maryland License.
• Active American Heart Association Basic Life Support (BLS) certification required.

Work Schedule:

• Employment Type: Per Visit
• Hours per Week: 4 hrs/ weekly
• Typical Daily Schedule: 2 full days per month / weekends
• Shift Type: Days
• Weekend Requirements: Saturday and Sunday
• Holiday Requirements: As needed

Pay Range:

$34.58 - $51.87

If the salary range is not displayed or if the position is Per Diem (with a fixed rate), salary discussions will take place during the screening process.

Under the Fair Labor Standards Act (FLSA), this position is classified as:
