Licensed Practical Nurse (LPN)

We are seeking a dedicated and experienced Licensed Practical Nurse (LPN) to join our team in a Long-Term Care/Skilled Nursing Facility setting. The ideal candidate will be responsible for providing high-quality patient care and ensuring the well-being of residents.

Key Responsibilities

• Deliver direct patient care in accordance with established care plans.
• Administer medications and treatments as prescribed by healthcare providers.
• Monitor patient health and report changes to the nursing staff or physicians.
• Assist with daily living activities, including bathing, dressing, and feeding.
• Document patient care and progress using the EPIC electronic medical records system.
• Communicate effectively with patients, families, and medical staff regarding patient care.
• Participate in patient care planning and support an environment of continuous improvement.

Shift Details

• Evening shifts from 3:00 PM to 11:30 PM and night shifts from 7:00 PM to 7:30 AM.
• Schedules include two evening shifts and two night shifts per week.
• Every other weekend and holiday rotation is required.

Qualifications

• Must have at least 1 year of experience as a Licensed Practical Nurse.
• Current Basic Life Support (BLS) certification is required.
• Experience in Long-Term Care or Skilled Nursing Facility settings is preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.

Additional Information

• Guaranteed 40 hours of work per week.
• Shifts may be 8 or 12 hours, with variability in the schedule.
• Weekend and holiday coverage is mandatory.
• Location-specific orientation will be provided.
• Common patient diagnoses include a general Long-Term Care population with some short-term care needs.
