Nurse Practitioner - Full Time State Traveler

Overview $7,500 Sign-On Bonus. Matrix Medical Network provides in-home clinical assessments to improve health and care for older adults and at-risk individuals. Responsibilities During a visit that lasts up to one hour, providers review and observe a member’s health status, medical history, medication adherence, social environment, and other risks. This assessment informs a comprehensive health plan and helps close care gaps. Key tasks include: Conduct adult/geriatric assessments, including medical history, diagnosis, treatment, health education, and referrals. Administer and order point-of-care and lab-based tests as needed. Collaborate with physicians, case managers, social workers, caregivers, and ancillary medical personnel. Coordinate follow‑up with the Primary Care Physician (PCP) for patient education and ongoing care. Provide services at the member’s residence in homes or skilled nursing facilities. Benefits Consistent schedule: Full‑time providers work Monday to Friday, 8:00 a.m.–5:30 p.m.; assessments are available Sunday–Saturday, 8 a.m.–7 p.m. Competitive base salary: $105,500 – $168,400 annually, based on market analysis and experience. Incentive program: Eligible for a monthly (uncapped) incentive; average $500 per month ($6,000 annually). Mileage reimbursement, medical, dental, vision, life insurance, PTO accrual, 401(k), paid holidays, continuing education time/reimbursement, employee referral bonuses. Educational support: Comprehensive onboarding, preceptor assignment, 1:1 feedback, free access to continuing education. Licensure support: Assistance obtaining additional state licensure and credentialing in neighboring states as needed. Qualifications Master’s Degree or equivalent experience plus successful completion of NP licensure. Current NP licensure in state of practice with prescription authority or ability to obtain prescriptive authority. Board certification by AANP, AACN, or ANCC in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialties. Current BLS, ACLS or CPR certification. One year of NP experience. Willingness to travel up to 80‑90% of the time, including overnight stays and weekends across the assigned state and neighboring states. Valid driver’s license for the state of residence and personal automobile insurance coverage required. Liability insurance coverage: Bodily injury $100,000 per person/$300,000 per incident; Property damage $50,000; Collision and comprehensive deductibles $1,000 or less. Strong computer skills and familiarity with employee health/medical record software. Excellent verbal and written communication skills.
