Homeside Rehab is seeking compassionate and motivated Occupational Therapist (OT) and Certified Occupational Therapy Assistant (COTA) to provide high-quality, individualized therapy services to patients in their homes in Florida.

Responsibilities
- Perform comprehensive occupational therapy evaluations.
- Develop individualized treatment plans based on patient needs and goals.
- Provide evidence-based occupational therapy interventions to improve functional independence and quality of life.
- Educate patients and caregivers on treatment plans and home exercise programs.
- Maintain accurate and timely electronic documentation.
- Collaborate with physicians, caregivers, and interdisciplinary team members to ensure coordinated care.
- Monitor patient progress and modify treatment plans as appropriate.
Qualifications
- Active Florida Occupational Therapy License (OT) or Certified Occupational Therapy Assistant (COTA) License (Required)
- Minimum one year of experience in Home Health, Outpatient, Skilled Nursing Facility (SNF), or related clinical setting preferred
- Strong clinical reasoning and communication skills
- Comfortable working independently in patients' homes
- Basic computer skills for electronic documentation
- Valid Florida driver's license
- Reliable transportation
