You will be directly working with patients, following a one‑on‑one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail‑oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus. Responsibilities Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles Provide attentive personal care and companionship Care for patients by changing bed linens, doing laundry, cleaning the home, and assisting with personal hygiene and grooming Prepare meals and assist with feeding when needed Run errands such as grocery shopping, picking up prescriptions, and attending appointments Maintain records of patient care, condition, progress, or problems to report observations to the supervisor Build positive relationships with patients by conversing, reading aloud, or engaging in meaningful activities to keep them mentally healthy and alert Provide medication reminders Assist with mobility, transfers, and light exercises Monitor patient safety and overall well‑being Assist with toileting and incontinence care when necessary Support patients with daily routines and independent living skills Escort clients to appointments or community activities when needed Provide light housekeeping and homemaker services to maintain a safe and comfortable environment Offer emotional support and companionship to clients and families Qualifications Home care experience preferred but not required Driver’s license required CPR certification required TB Test Car Insurance Must pass background check #J-18808-Ljbffr POWERFUL HEARTS HOME CARE LLC
