Case Manager Intake Specialist

Job Description About the Role: We are seeking a highly motivated Intake Specialist/Case Manager to support a dynamic, fast‑paced program with daily intake goals and continuous client engagement. This role has a strong intake emphasis, requiring efficiency, accuracy, and the ability to manage high client volume. The ideal candidate is goal‑oriented, organized, and experienced working with diverse populations, including individuals in housing and permanent supportive housing programs. Responsibilities Conduct client intake interviews, orientations, eligibility screenings, and initial assessments with accuracy and urgency. Review applications, records, and documentation to determine service needs and appropriate referrals. Establish client schedules, routines, and individualized service plans while coordinating supportive resources (including transportation). Monitor client progress by verifying attendance, evaluating needs, addressing barriers, and advocating for essential services. Maintain detailed, accurate, and compliant case records, notes, logs, and progress documentation. Participate in interdisciplinary team meetings to communicate client updates, challenges, and recommendations. Support discharge planning and continuity of care efforts. Prepare reports, track data trends, and assist with grant‑related or program documentation as needed. Qualifications BS/BA degree in Social Work, Human Services or a related field. Experience in housing programs and/or permanent supportive housing strongly preferred. Demonstrated experience in intake and case management with emphasis on high‑volume client processing. Strong organizational skills with the ability to meet daily performance goals. Excellent communication, documentation, and problem‑solving abilities.
