RN - Case Manager

Job Opportunity

We are seeking two experienced RN Care Managers for a 13-week assignment at our main campus located in York, ME. This is a full-time position with a schedule of 8:30 AM to 4:30 PM, requiring a weekend and holiday rotation. There is no on-call time for this position.

Job Responsibilities:

• Conduct comprehensive assessments of patient needs and identify required resources for smooth transitions to the next care setting.
• Collaborate with and lead an interdisciplinary team of healthcare professionals in daily care rounds, conferences, and care planning.
• Communicate effectively using written, verbal, and electronic systems, including hospital software applications and email.
• Engage with community organizations to secure durable medical equipment, health insurance authorizations, and transportation arrangements.
• Participate actively in managing the length of stay for inpatient populations.
• Maintain knowledge of hospital reimbursement methods and quality measures relevant to care management.

Qualifications:

• Must possess an Associate or Bachelor's degree in nursing.
• 3-5 years of clinical experience in medical/surgical nursing, home care, hospital case management, and/or previous utilization review experience is required.
• Experience in utilization review and discharge planning is essential.
• Certification from a nationally recognized case management or utilization review organization is preferred.
• Current Basic Life Support (BLS) certification is mandatory; Certification in Case Management (CCM) or Accredited Case Manager (ACM) is preferred.
