Field Nurse (RN)-Home Care

The Opportunity As a Field Nurse (RN) with Executive Home Care, you'll be vital to our patients' well-being, providing comprehensive nursing care and support in the comfort of their homes. This role offers professional independence, direct patient impact, and the satisfaction of building meaningful relationships. Key Responsibilities Comprehensive Patient Assessment: Conduct thorough initial and ongoing assessments of patients' physical, mental, and emotional health, identify needs, and develop individualized care plans. Medication Management: Assist with medication administration and ensure proper adherence to prescribed regimens. Personal Care Assistance: Oversee and provide guidance on activities of daily living (ADLs), such as bathing, dressing, and feeding, to ensure patient comfort and dignity. Vital Signs Monitoring: Regularly monitor and assess vital signs, reporting any significant changes to the appropriate healthcare professionals. Health Education & Support: Educate patients and their families on general health and wellness, disease prevention, and self‑care techniques to promote independence and optimal health outcomes. Documentation & Communication: Maintain accurate, timely, and detailed clinical documentation in electronic health records (EHRs). Communicate effectively with the interdisciplinary team, physicians, and families to ensure coordinated and holistic care. Care Coordination: Act as a liaison between patients, physicians, and other healthcare providers to ensure seamless transitions of care and access to necessary resources. Emergency Response: Respond to changes in patient condition and medical emergencies, initiating appropriate interventions and seeking medical assistance as needed. Adherence to Standards: Adhere to all professional nursing standards, ethical guidelines, and company policies and procedures, including HIPAA regulations. Professional Development: Participate in ongoing professional development and continuing education to maintain licensure and enhance clinical skills. Qualifications Valid and unrestricted Registered Nurse (RN) license in New Jersey . Associate's Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred. Minimum of 3 years of recent clinical experience as an RN , with at least 3 years in home care, long-term care, or a similar community-based setting . Current CPR certification (BLS required) . Valid driver's license, reliable transportation, and auto insurance. Excellent clinical assessment, critical thinking, and problem‑solving skills. Strong communication, interpersonal, and organizational skills. Ability to work independently and manage a caseload effectively. Proficiency in using electronic health records (EHRs) and other relevant technology. Compassionate, empathetic, and patient‑centered approach to care.
