Responsibilities
- Deliver PT services under supervision, implementing treatment plans for LTAC residents.
- Administer therapeutic exercises, gait training, ROM, and modalities as prescribed; monitor patient response.
- Assist patients with activities of daily living and mobility tasks per plan of care; support safe transfers.
- Document patient progress accurately in medical records; communicate changes to the supervising PT.
- Maintain clinical environment, equipment, and safety protocols; participate in care coordination with the interdisciplinary team.

Requirements
- Active Physical Therapy Assistant license in New Hampshire (or eligible for licensure in NH).
- Associate degree in Physical Therapy Assisting from an accredited program.
- Current Basic Life Support (BLS) or CPR certification.
- Prior LTAC, SNF, or rehabilitation setting experience preferred.
- Ability to work 36 hours per week in the stated shift; capable of functioning in a skilled nursing facility environment.
