Responsibilities
- Provide skilled nursing care in patients’ homes, including assessment, monitoring, and vital signs.

- Administer medications and injections per physician orders, within scope of practice.
- Develop, implement, and adjust care plans in collaboration with the supervising nurse and physician; maintain accurate documentation in the patient record.
- Educate patients and families on disease management, medications, safety, and home care routines; reinforce fall prevention and infection control practices.

Requirements
- Active Licensed Practical Nurse (LPN) license in Pennsylvania
- 2+ years of home health or home care nursing experience
- Proficient in wound care
- Ability to serve zip codes 19104 and 19139
- Availability to work the 8:00am–4:30pm shift and participate in a travel assignment within the service area
