Registered Nurse Intake Coordinator

Location: Escondido, CA (Onsite) Schedule: Monday- Friday Flexible ~ 8:00am - 4:30pm OR 9:00am - 5:30pm Pay: ~$50-$55/hour

Position Summary

Amergis Healthcare Staffing is seeking a Registered Nurse Intake Coordinator to support a community-based healthcare program serving individuals with complex medical and behavioral health needs. This role is responsible for reviewing referrals, coordinating admissions, conducting nursing assessments, and collaborating with interdisciplinary teams to ensure appropriate placement and continuity of care.

Key Responsibilities

• Review referrals and coordinate admissions from hospitals, health plans, and community partners.
• Conduct clinical screenings and assess medical and behavioral health stability.
• Review medical records, labs, and documentation to determine program eligibility.
• Monitor participant health status and report significant changes.
• Provide health education, medication support, and care coordination.
• Collaborate with providers, pharmacies, case managers, and external agencies.
• Assist with discharge planning and transitions of care.
• Maintain accurate and timely clinical documentation.
• Participate in care team meetings and quality improvement initiatives.

Qualifications

• Active California RN license required.
• ADN required; BSN preferred.
• Minimum 1 year of RN experience.
• Experience in admissions, case management, care coordination, community health, or behavioral health preferred.
• Current BLS certification (or ability to obtain).
• Bilingual Spanish preferred.
