Special Operations Nurse Case Manager / Case Manager Part-Time (National Capital Region) We are seeking a Special Operations Case Manager (PART TIME) who will provide a full range of professional health nursing principles, practices, and procedures in clinical settings to analyze the full scope of problems associated with providing appropriate, cost‑effective care to Department of Defense beneficiaries. Essential Duties & Responsibilities Collect, organize, record, and communicate data relevant to primary health assessments, including a detailed medical history, to develop time‑sensitive treatment plans that outline expected care delivery for selected case‑managed patients or populations. Assess patients via telephone using established protocols to provide appropriate and cost‑effective care. Establish priorities for patient care, monitor and evaluate progress toward stated goals to provide coordinated, efficient, and effective health care to beneficiaries. Oversee discharge‑planning activities to ensure ideal timing and sequencing of patient care. Provide professional assistance to health‑care finders to identify patients’ needs for referrals to appropriate health‑care providers or facilities. Negotiate, write, finalize, and administer product/service agreements to ensure mutual compliance in meeting care goals. Use pre‑established utilization review criteria to recognize and report potential quality and risk‑management issues to improve practice and ensure compliance. Apply VA/DOD Clinical Practice Guidelines to ensure evidence‑based standardized processes of care for military beneficiaries. Understand the healthcare financial environment to ensure optimal stewardship of population‑health resources. Use research tools to critically evaluate existing practices, opportunities, and outcomes. Manage information systems and technology to manage individual or population health, improve practice, and communicate outcomes to stakeholders. Use computers and software applications for data collection, tracking, rendering, and analysis. Design and present computer‑generated outcome analysis information to provide timely, effective peer and higher‑level educational programs or project briefings. Understand pharmaceuticals, their effects, side effects, and complications to gauge their impact on patients and recovery. Required Education, Experience, Skills & Training U.S. Citizenship. Secret or Top Secret Clearance, or ability to obtain one. Current licensure: full, active, and unrestricted license as a Registered Nurse or Licensed Clinical Social Worker in one of the following U.S. State, District of Columbia, Guam, Puerto Rico, or U.S. Virgin Islands. Minimum of two (2) years of nursing experience or case‑management experience. Certification as a case manager by a recognized certifying organization (e.g., Commission for Case Management Certification, American Case Manager Association, or American Nurse Credentialing Center), or obtain certification within the first year of employment. Knowledge of patient education principles and management of complex medical, psychosocial, and financial problems to enhance adherence to individualized treatment plans. Ability to seek and incorporate feedback from peers, professional colleagues, clients, and outcomes research. Skill to consult with health‑care providers at all levels and negotiate with outside providers for services and products. Possibility to receive and maintain clinical practice privileges and to keep credentialing requirements in good standing at a local MTF. Fluent oral and written communication skills in English. Skill in team coordination, facilitation, and communication to facilitate information exchange and goal achievement. Knowledge of local, state, and federal laws, professional code of ethics, and practice standards. Maintain current certification in Basic Cardiac Life Support (BCLS) by the American Heart Association; provide proof of all training, certifications, and documentation. Compliance with Safety, Health, and Environmental plan; Quality Assurance plan; regular attendance; completion of required training; and performance of other assignments as required. Desired Qualifications & Experience Candidate has previous military background or experience. Basic Compensation $49.31 per hour – $54.64 per hour, based on working location, knowledge, skills, and experience.
