Job Summary Employment Status: Variable / Flex Shift: Day (United States of America) Facility: 390 S Main St - Rocky Mount, Requisition Number: R153269. Home Health Nurse- PRN/Flex (Open). As part of the healthcare team, the Home Health Nurse assumes primary accountability for nursing coordination and provision of individualized care for patients of all ages. Utilizes delegation and critical thinking skills to provide quality service through a team‑oriented, collaborative approach to patient care. The nurse assesses, plans, implements, and evaluates patient care to achieve optimum patient outcomes, performs age‑specific admission and ongoing assessments, develops and revises interdisciplinary plans of care, administers treatments and medications accurately, and incorporates patient/family education. The nurse also performs clinical leadership, quality improvement, resource management, problem‑solving, and roles such as orientation of new employees. Physical requirements include lifting and standing; the practitioner may be exposed to infectious diseases, bio‑hazardous and hazardous materials, and combative patients. Some positions may be required to take call and may provide care at other Carilion facilities. Career Advancement Program RNs are eligible to participate in the Career Advancement Program (CAP) to attain various levels of advancement, with levels determined by achievements outlined in the CAP Scoring Sheet as part of the RN performance appraisal. Qualifications Education: Graduate of a school of professional nursing. Experience: One (1) year experience in hospice, home health, oncology, or medical‑surgical nursing required. Licensure, certification, and/or registration: Currently licensed as a Registered Nurse in the Commonwealth of Virginia; for Home Health, must have a valid Virginia Driver’s License and pass a Criminal Record Check as required by state law. Life Support: AHA BLS‑HCP required. Other Minimum Qualifications: Must successfully complete orientation and competency requirements, demonstrate keyboarding skills for automated computer systems including electronic medical records, and be able to meet essential job functions.
