Responsibilities Must complete a minimum of 36 weeks of Clinical Fellowship, supervised and mentored by a licensed, ASHA certified SLP. Must complete all requirements for provisional licensure (if applicable to the state), practice in accordance with state licensure requirements and professional standards for the duration of the provisional license and Clinical Fellowship. If no provisional licensure in the state, must meet CMS supervision requirements for Medicare patients during the Clinical Fellowship. Communicate status of Clinical Fellowship and licensure to direct supervisor and Director of Rehab in a timely manner. This includes status of necessary requirements, completion of required documentation for Clinical Fellowship, and completion of licensure documentation and application as well as receipt of CCC and permanent license. Must not provide supervision to students during the Clinical Fellowship. Meet productivity standards for the rehab gym, or as directed by the Director of Rehab/Clinical Operations Area Director. Attend and contribute to patient care, staffing conferences and other related meetings. Make referrals to other disciplines or agencies to address comprehensive patient needs. Consult with other members of the rehabilitation team to select the most appropriate therapeutic regimen consistent with the needs and capabilities of each patient. Adapt program and/or plan of care according to the needs of the individual patient. Promote a safe environment and enforce elimination of fire and safety hazards. Order supplies and equipment as necessary. Clean, maintain, identify, and report need for repair of equipment, as necessary. Put Patient Service first, ensuring residents and families receive highest quality of service in a caring and compassionate atmosphere that recognizes the individual's needs and rights. Screen, examine, and evaluate patients, including history, systems review, and application of appropriate tests and measures; synthesize information and interpret examination findings to establish a diagnosis, identify impairments, determine predicted level of improvement and time required to achieve it, identify precautions/contraindications, and design a plan of care. Develop appropriate treatment goals and methods in collaboration with the patient and caregivers, implement the speech‑language pathology treatment plan, and complete all related documentation and record keeping regarding these services. Services include but are not limited to application of therapeutic techniques in areas such as: cognitive‑communication speech intelligibility oral motor skills swallowing disorders/dysphagia perceptual abilities orientation memory pragmatics psychosocial expression functional communication Coordinate acquisition of adaptive communication devices and instruct patients in their use. Facilitate identification of hearing deficits and appropriately refer for assessment, diagnosis, rehabilitation and/or personal amplification. Select appropriate activities suited to the individual's physical and mental capacity, intelligence level and interest. Instruct patient in the skills and techniques required for participation in therapeutic activities, and evaluate patient progress, attitudes, and behavior related to rehabilitative potential. Perform routine reexamination as needed/required to modify/progress plan of treatment. Perform other related duties as required. Qualifications Must possess a master's degree in Speech‑Language Pathology, Communication Disorders, Communicative Disorders or similarly titled area that is consistent and acceptable to the American Speech‑Language‑Hearing Association.
