Overview

Community Health Partners is seeking a full-time Advanced Practice Provider (APP) to join the Neurocritical Care team. In this role, the APP will provide comprehensive care for patients with critical neurologic illness, including acute stroke, intracranial hemorrhage, status epilepticus, and patients recovering from neurosurgical and neurointerventional procedures, in collaboration with the interdisciplinary care team. Responsibilities include conducting detailed clinical assessments, developing and managing treatment plans, prescribing appropriate therapies, and performing critical care bedside procedures. The APP will support patients throughout the critical care continuum and ensure high standards of safety and quality. This position follows a 7-on, 7-off schedule with day and night shifts. Night shift responsibilities are assigned based on experience and training.

Our Neurosciences department now includes the Bob Smittcamp Family Neurosciences Institute, a five-story, 60,000-square-foot facility on the Community Regional Medical Center campus that opened in May 2026. Funded by $30 million in community donations, the Institute brings together more than 60 physicians and 30 advanced practice providers across stroke and neurovascular care, epilepsy, neuro-oncology, traumatic brain and spine injury, movement disorders, and neuromuscular disease. Community Regional is certified by The Joint Commission as an Advanced Comprehensive Stroke Center, the highest level of stroke certification and the only one in the five-county region, with 24/7 access to on-site neurosurgical expertise. Our 30-bed Neurocritical Care Unit has also earned the AACN Beacon Award for Excellence at the Gold level.

In this role, you will:

• Provides direct assessment, evaluation and primary patient care, complying with medical protocols and procedures.
• Provides direct patient management in collaboration with physicians; participates in daily rounds, patient assessments and care plan development.
• Orders, interprets and evaluates diagnostic tests to identify and assess patient's healthcare needs. Furnishes medications based upon written protocols.
• Performs critical care bedside procedures, including central venous catheter placement, arterial line placement, lumbar puncture, and endotracheal intubation as credentialed.
• Ensures that patients are counseled regarding treatment plans and the use, dosage and possible side effects of medications.
• Makes referrals to appropriate providers when patients require subspecialty medical attention beyond the area's scope of service.
• Documents and maintain precise and complete records of patients.
• Distinguishes between normal and abnormal findings.
• Provides education, training, information and consultation services to interdisciplinary team, patients, family and caregivers.

Accountabilities

1. Deliver a high standard of medical care to individuals in the community.

2. Work alongside staff members as a part of an integrated, interdisciplinary team.

3. Develop and deliver a model of care that supports the wellness of our patients and staff.

4. Assess, diagnose, and develop treatment plans for patients.

5. Effectively communicate treatment plans with patients, family members, and other medical staff members.

6. Assist and advise staff members on challenging cases, as needed.

7. Refer patients to specialists or other practices, as appropriate.

8. Maintain immaculate documentation of all procedures and practices.

9. Performs other duties as assigned.

Qualifications

Education and Experience Minimum Requirements:

• Completion of an accredited Nurse Practitioner or Physician Assistant program required
• Credentialed by the CMC Medical Staff Office as an Advance Practice Provider

Licenses and Certifications:

If qualifying as a Nurse Practitioner, the following is required

• NP - Current State of California Nurse Practitioner License required
• NPF - Nurse Practitioner Furnishing Number required

If Qualifying as a PA, the following is required: 

A current certification by the National Commission on Certification of Physician Assistants (NCCPA).
