REGISTERED NURSE CASE MANAGERA Path of Care Hospice in Norman, OK is seeking a full‑time RN Case Manager to provide compassionate, high‑quality nursing care to patients in their homes. We work hard to create a supportive, growth‑oriented environment where employees can build meaningful, long‑lasting careers.What You'll Do as an RN Case ManagerAs an RN Case Manager, you ensure patients receive exceptional nursing care and support throughout their hospice journey. Your responsibilities include:Completing initial assessments within 48 hours of hospice electionWorking with the attending physician and interdisciplinary team to complete comprehensive assessmentsDeveloping, coordinating, and initiating individualized plans of careConducting ongoing assessments and updating care plans based on changes in patient conditionAddressing physical, psychological, social, and spiritual needsCommunicating regularly with physicians and the care team to maintain continuity of careMaintaining accurate, timely documentation to meet regulatory requirementsProviding leadership by instructing caregivers and volunteers and supervising LPNs and hospice aidesPracticing infection control and reporting any signs of abuseProviding direct patient care as needed and assisting with discharge planningParticipating in professional development, community programs, and agency initiativesYour work ensures patients receive compassionate, comprehensive care while supporting families through a challenging time.QualificationsActive Oklahoma Registered Nurse licenseAt least 1 year of clinical experienceReliable transportation and vehicle liability insuranceHospice, home health, critical care, or emergency room experience preferredStrong interpersonal and communication skillsAbility to adapt to changing circumstances, mentor others, and work with compassion and patienceCommitment to integrity, critical thinking, and teamworkWork ScheduleFull‑timeMonday-Friday, 8:00 AM - 5:00 PMRotating on-call and weekendsReady to Join Our Nursing Team?Our application process is quick and easy.
