Job Opportunity – General Medicine Provider – Arizona

Start Date: Ongoing, PRN basis
Location: Multiple locations throughout Arizona
Compensation: $800 per half-day (4 hours or less) / $1,600 per full day (5+ hours); $650 one-time completion bonus
Schedule: Flexible PRN scheduling with half-day and full-day options available; locations include Tuba City/Chinle (1 day per week), Nogales (2 days per week), and Payson (half-day per week)

• Interview-based examinations with physical assessments including range of motion and muscle strength testing
• No prescribing or treatment responsibilities required

Requirements:

• Active and unrestricted state license in Arizona
• Board certification required
• AANP certification required
• Background in Family Medicine, Internal Medicine, Orthopedic Surgery, or General Medicine
• Proficiency in METs testing, respiratory and cardiovascular evaluation and diagnosis
• Must own a laptop (non-Mac/Apple)
• Willingness to complete DMA training and certification
• Must reside within 50-mile radius of assignment location (40-mile radius for Nogales)
• Self-Query NPDB dated within 30 days
• If utilizing own office: Must meet accessibility requirements and provide compliant facility photos
