Schedule

Full-time, Monday to Friday, 8:00 AM - 4:30 PM with weekend rotation required every fourth weekend.

Job Description

We are seeking a dedicated Registered Nurse for our Home Health unit. The ideal candidate will be committed to providing quality care to adult and geriatric patients in various home settings. The role includes performing assessments, care planning, medication management, and coordinating patient care.

Responsibilities

• Conduct patient assessments and develop individualized care plans.
• Administer medications and manage patient care effectively.
• Provide education and support to patients and families.
• Perform wound care and other specialized nursing tasks as required.
• Ensure adherence to infection control precautions and safety standards.
• Document patient care in accordance with policies and procedures using EPIC.

Required Experience / Certifications / Licensure

• Minimum of 2 years of nursing experience in home health or a relevant setting.
• ASN or BSN required.
• BLS certification required.
• OASIS experience required.
• Active RN license in Pennsylvania required.
