About the Role:
The Renaissance is seeking PRN MDS nurse. The MDS Nurse plays a critical role in ensuring the accurate and timely completion of the Minimum Data Set (MDS) assessments, which are essential for the delivery of quality care in long-term care facilities. This position involves comprehensive evaluation of residents' health status, coordinating with interdisciplinary teams to develop individualized care plans that meet regulatory requirements and improve patient outcomes. The MDS Nurse is responsible for maintaining compliance with federal and state regulations by accurately documenting clinical data and facilitating communication between nursing staff, physicians, and administrative personnel. This role requires a detail-oriented professional who can analyze complex clinical information and apply it to care planning and reimbursement processes. Ultimately, the MDS Nurse contributes to enhancing the quality of life for residents by ensuring their care needs are thoroughly assessed and addressed through effective care coordination and documentation.
Minimum Qualifications:
- Current and valid RN or LPN license in the state of employment within the United States.
- Minimum of two years of clinical nursing experience, preferably in long-term care or skilled nursing facilities.
- Demonstrated knowledge of MDS assessment processes and federal/state regulatory requirements.
- Strong understanding of clinical documentation and care planning principles.
- Proficiency in electronic health records (EHR) and MDS software systems.
Preferred Qualifications:
- Certification as a Certified MDS Coordinator (CMS) or similar credential.
- Experience with quality improvement initiatives and data analysis in a healthcare setting.
- Familiarity with Medicare and Medicaid reimbursement methodologies related to long-term care.
- Excellent communication and leadership skills to effectively train and support interdisciplinary teams.
- Prior experience working in a multi-disciplinary team environment within a long-term care facility.
Responsibilities:
- Conduct thorough assessments of residents using the Minimum Data Set (MDS) tool in accordance with regulatory guidelines.
- Collaborate with interdisciplinary teams including nursing, therapy, social work, and medical staff to develop and update individualized care plans.
- Ensure accurate and timely completion of MDS documentation to support clinical decision-making and reimbursement processes.
- Monitor residents’ clinical progress and update assessments as necessary to reflect changes in condition or care needs.
- Provide education and training to nursing staff on MDS processes, documentation standards, and regulatory compliance.
- Review and analyze clinical data to identify trends, potential issues, and opportunities for quality improvement.
- Maintain compliance with federal and state regulations related to MDS assessments and reporting requirements.
- Serve as a resource for staff and administration regarding MDS guidelines, coding, and documentation best practices.

The MDS Nurse utilizes clinical assessment skills daily to evaluate residents’ health status accurately and comprehensively. Strong analytical skills are essential for interpreting clinical data and ensuring that documentation meets regulatory standards and supports appropriate reimbursement. Effective communication skills enable the nurse to collaborate with interdisciplinary teams, educate staff, and advocate for residents’ care needs. Proficiency with electronic health records and MDS software facilitates efficient data entry, retrieval, and reporting, which are critical for compliance and quality assurance. Leadership and organizational skills are applied to manage multiple assessments, coordinate care plans, and contribute to continuous quality improvement efforts within the facility.
