Crossbridge Hospice is seeking an RN Case Manager to deliver end-of-life care in patients' homes or community settings. You will supervise hospice aides, coordinate with the triage team, and ensure comprehensive care aligned with the four M’s: What Matters, Medications, Mentation, and Mobility.
