Who We Serve
North Bay Regional Center provides advocacy, services, support, and care coordination to over 13,000 children and adults diagnosed with intellectual and developmental disabilities and their families in Napa, Sonoma and Solano counties. North Bay Regional Center is a nonprofit private corporation that contracts with the Department of Developmental Services to coordinate services and support for individuals with developmental disabilities.

Note: Bilingual Spanish/English skills are preferred.
DEFINITION:
Responsible for the full range of case management duties including cases requiring intensive case management, crisis intervention, and high levels of expertise. Develop and implement service plans for individuals served. Responsible for consultation to Service Coordinators on cases requiring intensive and/or specialized case management.
NATURE OF WORK:
Under the direction of a Case Management Supervisor or the Legal Supervisor, the Senior Service Coordinator manages the delivery of services to individuals served by the regional center with intensive physical or emotional needs, as well as individuals in crisis; ensures Individual Program Plans are developed and objectives are monitored; assists individuals served in their use of public and private resources before using regional center funds to purchase services; monitors vendor services and verifies services are delivered in accordance with the purchase of service authorizations; evaluates the effectiveness of Individual Program Plans; ensures appropriate community resources are made available to the individuals served; conducts annual reassessments of the developmental status of individuals served in conjunction with the interdisciplinary team; ; conducts assessments for individuals served. Shall act occasionally, as assigned, in a lead capacity, e.g., Officer of the Day or Acting Supervisor.
The Senior Service Coordinator shall, as assigned, provide consultation to Service Coordinators in the provision of crisis and intensive services to individuals served by the regional center.
SERVICE COORDINATOR - FORENSIC SPECIALIST:
This position specializes in working with individuals with ongoing court involvement, individuals in jail, individuals residing at Porterville Developmental Center, and individuals residing in locked facilities. This position includes regular court reporting requirements and collaboration with the court system. This position assists with transitioning individuals out of locked settings statewide and may require travel outside of the NBRC catchment area. This position facilitates meetings with the Department of Developmental Services (DDS) to coordinate transition planning for individuals moving from Porterville Developmental Center and IMDs, including College Hospital Cerritos.
SENIOR SERVICE COORDINATOR - COMPLEX CASELOAD:
This position specializes in working with individuals that meet the 1:25 complex case caseload criteria. This position may require travel outside of the NBRC catchment area.
The individuals served who meet the 1:25 criteria include the following;
· Receiving regional center-funded mobile crisis services by a department-approved vendor or has received those services within the past six months.
· Receiving state-operated crisis assessment stabilization team services or has received those services within the past six months.
· Placed in a community crisis home, as defined in Section 4698.
· Placed in an acute crisis home operated by the department, pursuant to Section 4418.7.
· Placed in a locked psychiatric setting or has been placed in a locked psychiatric setting in the past six months.
· Placed in an institution for mental disease, as described in Part 5 (commencing with Section 5900) of Division 5.
· Placed out of state as a result of appropriate services being unavailable within the state, pursuant to Section 4519.
· Placed in a county jail and eligible for diversion pursuant to Chapter 2.8 (commencing with Section 1001.20) of Title 6 of Part 2 of the Penal Code or found incompetent to stand trial as described in Section 1370.1 of the Penal Code.
EXAMPLES OF WORK:
Attends interdisciplinary team meetings; conducts meetings with individuals served and their family members; coordinates the development of Individual Program Plans; visits and reviews community programs; coordinates transition of individuals to community residential programs; arranges move-in of individuals into residential programs; provides intensive follow-up case management activities, completes written forms and reports; consults with Service Coordinators on the development of plans and interventions for individuals in crisis or in need of intensive case management services.
Encourages individuals served and their families to become as independent as possible by assisting them in coordinating and utilizing available resources. Formulates client service plan and annual revisions; completes Client Developmental Evaluation Reports (CDER); recommends necessary services. Completes written reports and forms; participates in outreach programs; assists community groups in meeting with individuals served and their families who need assistance in obtaining services from other agencies; assists individuals in securing out of home living options; monitors individuals in out-of-home settings. Travels out of area to visit individuals in residential settings.
A Senior Service Coordinator shall, as assigned, work with individuals and family members who have severe interpersonal disturbances, emotional disorders, or other special challenges which require a demonstrated use of problem solving and active listening skills, as well as clear communication and collaboration with individuals, families, and groups.
A Senior Service Coordinator shall, as assigned, work with individuals with severe social, emotional and physical needs, and prepare court-ordered evaluations and reports as required. A Senior Service Coordinator shall, as assigned, provide case management for families experiencing situational stress which could lead to a crisis.
A Senior Service Coordinator shall advocate for individuals served and their families to ensure they receive entitled services. Case assignments to Senior Service Coordinators will be made according to individual skills and training.
Requirements
MINIMUM EDUCATION AND EXPERIENCE:
Completion of a Master's Degree in Social Work or similar field which offers content related to developmental disabilities and two years' experience in the field;
OR
Bachelor’s degree in Social Work or similar field, and two years' experience in a professional position at NBRC.
Minimum experience requirements may be waived based on additional qualifications, i.e., fluency in Spanish.
Minimum education requirements may not be waived.
KNOWLEDGE AND ABILITIES:
- Extensive knowledge of developmental disabilities, case management and assessment functions; ability to work with community and state agencies; possession of excellent interpersonal skills. Knowledge of normal and abnormal individual and group behavior, human growth and development; dynamics of family relationships counseling and crisis intervention; ability to work independently and exercise good judgement in making decisions; ability to work as part of interdisciplinary team; ability to stimulate the cooperation of individuals served, their families, community agencies, and Regional Center staff; ability to maintain case records in a clear and concise manner.
- The successful candidate should have knowledge of local residential programs and specialized programs around the state.
SPECIAL REQUIREMENTS:
- Fluency in a language in addition to English is desirable.
- Ability to provide personal automobile transportation, or access to another form of reliable transportation, during and after working hours.
- Must be able to independently regularly travel to all NBRC offices within NBRC’s catchment area to attend and participate in community events, which may include evenings and weekends.
- If operating own vehicle, provide minimum automobile liability insurance as required by California Law.
- Flexibility in working hours is required to attend and participate in community events.
- This position will be primarily based in the office; however, a hybrid work arrangement may also be available.
ESSENTIAL FUNCTIONS:
- Visit clients in their homes and day programs, hospitals and jails. This may include maximum correctional facilities.
- Travel out of the NBRC catchment area to visit clients in residential settings.
- Prepare and submit accurate and punctual documentation to courts.
- Communicate effectively in person, on the telephone and in writing.
- Organize, prioritize, and coordinate work activities.
- Read, interpret, and follow complex policies, procedures, laws, and regulations.
- Interact tactfully and effectively with those contacted in the course of the work.
- Perform work under stressful conditions, as required.
- Use initiative and sound, independent judgement within established guidelines.
- Maintain current workload with accurate and complete documentation.
- Maintain a minimum average of 380 units of TCM/ID notes per month.
- Attend meetings in and out of the office with punctuality.
- Conduct meetings, interviews, and evaluations; interpret and document the information.
- Represent the agency's philosophy, policies, and standards in a professional manner in the community and in the office.
- Consult with staff, vendors, and community agencies regarding state and federal programs and regulations.
- Monitor and evaluate the quality of programs which serve people with developmental disabilities.
- Make decisions with a minimum of direction.
- Analyze and find effective solutions to problems.
- Understand and utilize sound case management and counseling principles.
- Adhere to a reliable work schedule, i.e. punctuality in reporting to work.
- Accurate and timely recording on time sheet and mileage forms.
- Notification to supervisor of schedule changes, i.e., use of accrued time-off benefits.
- Maintain a high level of personal and professional conduct.
- Other duties as assigned.
