Pay: $2,442.40 per week


Shifts: M-F 8 AM-5 PM, on call once a month for one weekend (Sat/Sun)

Key Responsibilities of the RN - Case Management - Travel

• Perform comprehensive assessments of high-risk members
• Collaborate with primary care providers to implement personalized care plans
• Address cost-efficient, appropriate levels of care and reassess continuously
• Coordinate non-medical support for compliance with treatment plans
• Manage patient care transitions effectively
• Engage specialty resources for optimal resolutions
• Maintain detailed records of clinical, functional, and fiscal outcomes
• Promote health and prevent illness through proactive measures
• Act as a patient advocate, ensuring privacy and confidentiality

Requirements of the RN - Case Management - Travel

• Associates or Bachelor's Degree in Nursing or related field
• Current, unrestricted state Registered Nurse license (GA or compact)
• Minimum of 30 benchmarks per week
• Proficiency in clinical skills including wound vac, PICC line, ostomy care, IV antibiotics, wound care
• Case Management certification
