Job Summary

The Ambulatory Clinical Pharmacy Specialist provides ambulatory clinical pharmacy services as a credentialed and privileged member of the healthcare team. The role works under collaborative practice agreements to optimize therapeutic outcomes, improve medication access, and reduce the overall cost of care. Responsibilities include comprehensive medication management, chronic disease management, specialty medication care coordination, patient education, and collaboration with multidisciplinary care teams.

• Provide ambulatory clinical pharmacy services under collaborative practice agreements.
• Conduct patient visits for medication management, medication reconciliation, laboratory monitoring, counseling, and disease-state management.
• Collaborate with providers and care teams to implement evidence-based medication interventions.
• Support patient access and affordability through prior authorizations, financial assistance, and benefit investigations.
• Provide clinical oversight and monitoring for patients receiving specialty medications.
• Educate patients, families, and healthcare team members on medication therapy.
• Support pharmacy students, interns, and residents.
• Participate in quality improvement, research, policy development, and committee initiatives.
• Document clinical interventions and outcomes in the electronic health record.

• Bachelor's degree in Pharmacy.
• Active pharmacist license.
• Board of Pharmacy Specialties certification.
• BPS certification in Pharmacotherapy, Ambulatory Care, Cardiology, or Geriatrics .
• At least one year of clinical pharmacy experience, including residency experience.
• Strong communication, analytical, and problem-solving skills.

• Master's degree or PharmD in Pharmacy.
