Mobile Nurse Practitioner (CareMore - West Covina, El Monte, Whittier, Pico Rivera, & East LA, CA)

Job Location West Covina, El Monte, Whittier, Pico Rivera, & East Los Angeles, CA Overview CareMore Health is a field‑based role requiring travel to various clinical sites and patient locations across the San Gabriel Valley and East Los Angeles service areas. Nurses will provide primary, acute and/or specialty healthcare services for patients in home or virtual settings. Responsibilities Provide a range of primary, acute and/or specialty healthcare services for patients in home or virtual settings. Where state requirements do not impose collaborative agreements, act as primary care provider. Order, interpret and evaluate diagnostic tests to identify and assess patient’s clinical problems and health care needs. Provide health instruction, counseling and education on disease prevention and healthy lifestyle choices. Collaborate with physicians, nurses and other healthcare professionals to treat patient needs as part of an interdisciplinary care team. Formulate and document care plans; prescribe medication or other treatments. Participate in an on‑call program, which may include after‑hours, weekend and holiday calls and visitations. Service Areas San Gabriel Valley: Pasadena, West Covina, ElMonte East LA: Whittier, Pico Rivera, Downey Requirements MS in Nursing. Minimum of 1 year of related clinical experience for a nurse practitioner or equivalent combination of education and experience. Current, active, valid and unrestricted RN license and NP license in Nevada. Successful completion of a tuberculosis test. Benefits & Compensation Sign‑on bonus: $10,000.
